Our Insurance Carriers
We accept many insurance plans but not all. And, within a particular insurance carrier, we may accept some plans but not others. Each plan sets their own copay limits and coverage differs with each provider. It is the patient's responsibility to know what is covered by their insurance carrier and plan. The patient is responsible to pay for all amounts not fully covered by their insurance carrier and also for all uncovered fees.

Insurance carriers offer mutiple plans. Services covered by your plan may vary and change peridocially. It is your responsibility to know what services your plan covers. For example, some plans cover physical exams only every other year while others allow them annually.
